Welcome to Cazenovia College!
This web section page will help familiarize you with our Campus Health Services. Our goal is to provide high quality, personalized care and health teaching, giving students the skills to make healthy lifestyle choices. The Health Office is located at 10 Seminary Street across from Hubbard Hall.
The office is staffed by nurse practitioners and registered nurses, who provide a broad range of health services and convenient hours. After-hours emergencies are coordinated on an individual basis by the residence hall director on duty. Should you require medical attention beyond the limits of the College Health Office, you will be referred to our consulting family practice office, Heritage Family Medicine, located on Albany Street in Cazenovia.
Before coming to campus you are required to have:
- a pre-college physical (transfer students may use their original college physical)
- proof of immunizations
- proof of health insurance
- a signed form regarding meningitis awareness
The required forms must be completed and sent to the Health Office by August 15 (by January 10 for spring term entrants).
All Cazenovia College students are required to carry health insurance. If you already have insurance, just sign the enclosed waiver form and attach a copy of the front and back of your card.
A Basic Student Insurance Plan is available for purchase after May 1 if you are not currently covered or would like supplemental coverage. Information on the plan is available from the carrier (https://www.gallagherstudent.com/cazenovia
An Enhanced Plan is also available through Gallagher Koster. Some insurance companies require that you change your Primary Care Provider (PCP) to someone in this area. Doctors who may be on their lists include Dr. Gerald Edwards (our consulting physician) of Heritage Family Medicine, Chenango St. in Cazenovia. You may also want to ask the insurance company if they require proof of enrollment as a full-time college student to continue coverage. The Registrar (315-655-7888) can provide you with a letter to send them once the fall term begins and after classes have started.
The Health Service staff aims to provide you with the best possible health care. During the summer, our hours are 9 a.m. to noon, Monday through Friday. Voice mail is also available when the office is closed.
Summer College Students are encouraged to have forms completed and returned by July 1.
